Appointments
In one to three working days, however, we will try and send it to you by the next day.
Provided they are agreeable, yes you can. The referral and appointment need to be for whoever is having the treatment (IUI or IVF), regardless of whose super is being accessed.
Your partner is of course welcome to attend the appointment with you.
A psychiatric assessment report outlines your history, diagnosis and suggestions for your treatment (psychological intervention, medication etc) so that your GP can assist with this.
Any treatment suggestions will be discussed with you during your appointment so that you can organise a follow-up appointment with your GP.
The superannuation access form is an ATO standard form. You can download it here.
Assessments are approximately 60 minutes.

You will require a browser (i.e. Chrome, Edge or Firefox), and a PC, smartphone or tablet. You will be given the option to either launch Zoom, download the Zoom app or connect via a browser page.
Yes, a cancellation fee applies if you cancel within 48 hours of the appointment date. This will be added to the cost of the next appointment you book.
The number of appointments and who should attend the appointment is a source of confusion for many clients.
How many appointments are needed, for a single CRS application, is dependent on whether the invoices they submit to the ATO are in more than one person's name.
One Person having a procedure
- If only one person is having a procedure, and therefore all invoices will be in a single name, then one appointment is needed. This is regardless of whether the person is in a couple or not.
- If they have separate invoices, two appointments will be needed, one for each person.
- If they have an invoice in only one person's name, only one appointment is needed. This should be for whoever is having the most expensive procedure.
ATO
To apply to the ATO you must include relevant itemised quotes or invoices:
- on the medical provider's letterhead dated
- no more than six months old for quotes
- no more than 30 days old for unpaid invoices
The reports supporting your applicaton must be:
- completed no more than six months before you submit your application.
- signed, dated and submitted with your application.
- completed by two doctors involved in your care, at least one of which must be a specialist and submitted with the documents listed above.
If you are applying to pay for medical treatment expenses you must:
- provide two separate medical reports. One is usually from your GP, and the other must be from a registered medical specialist
- the registered medical specialist must be specialised in the field of treatment for the patient
- the ATO will only consider one treatment per application
The ATO can only approve the application if you:
- meet the eligibility requirements of the compassionate ground you’re applying for
- can’t afford to pay the expenses without accessing your super e.g. by obtaining a loan or using your savings
- are or have been a citizen or permanent resident of Australia or New Zealand
- provide all required supporting evidence and invoices/quotes
The amount of superannuation you can withdraw is limited to what you reasonably need. It is paid and taxed as a normal lump sum, this is generally at a rate up to 32% but can differ depending on your circumstances. Your super fund will automatically deduct the tax from your super account, in addition to the approved release amount – the amount you receive from the release will not be affected.
The ATO medical report form is an ATO standard form. You can download it here. We have the form on our system therefore you do not need to send us a blank copy
The ATO can decline an application for several reasons. This is often a trivial issue (e.g. failing to date your paperwork) or a word or two that needs altering.
- The ATO will send you a letter advising why your application was declined.
- If you don’t understand the decision, you can contact the ATO for further explanation.

No. In an emergency situation, it is best to call 000 or attend your local Emergency Department.
Assistance in less urgent situations can be obtained from your local community mental health team (contacted via your local hospital) or GP.
We do not write prescriptions. If required, we request that prescriptions be written by your GP as they will be the primary care provider.
Well, the simple answer to that question is 'yes', provided you meet the eligibility requirements and provide all the necessary documents to confirm you are eligible to the ATO.
The second question you should be asking is ‘and can I make the application myself?’ This is a really important question to ask because the answer is most definitely ‘yes!’
